Basic Life and AD&D Coverage
You are automatically enrolled in basic life and AD&D coverage at no cost to you.

Life and AD&D insurance is designed to provide protection for you and your family against loss of income due to death.

You are automatically enrolled in basic life and AD&D coverage for $50,000 at no cost to you.

If you wish to increase your life and AD&D insurance amounts, you may purchase additional coverage.
You have the option to purchase supplemental coverage for you, your spouse/domestic partner, and your child(ren).
As a new hire or during open enrollment, you may elect voluntary coverage through Prudential:

For yourself – You can elect coverage in $10,000 increments up to a maximum of $400,000. The coverage has a Guaranteed Issue amount of $50,000 for employees who are less than 65 years of age.

For your spouse/domestic partner – You can elect up to 50% of your covered amount in increments of $5,000 up to a maximum of $200,000. The coverage has a Guaranteed Issue amount of $25,000 for your spouse/domestic partner who is less than 65 years of age.

For your children – You can elect a minimum of $1,000 and in increments of $1,000 up to a maximum of $10,000 per child. All coverage amounts are guaranteed.
If you wish to increase your voluntary life insurance after your initial eligibility or above the Guaranteed Issue amount, you will be required to submit Evidence of Insurability (EOI) for review and approval by Prudential. Please complete the EOI process as soon as possible and provide any additional information requested by Prudential within 60 days.
